The first step towards a beautiful, healthy smile is to schedule an appointment. Please contact our office by phone 312-332-4424 or complete an online appointment request.
We do our best to accommodate busy schedules so we do ask for a minimum 24 hour notice for cancellation of all standard appointments to avoid a cancellation fee. However, some may require longer appointments due to the nature of treatment and we do require a 48 hour notice for those individuals. The advance notice allows us to contact others who would like to come for those reserved times. We do appreciate your cooperation with this policy.
Patient Forms We are delighted to welcome you to our practice and are pleased that you chose us to serve your dental needs. If you are a new patient to our office, please click here or on the link below to complete our new patient bundle with forms that will need to be filled out when you arrive at our office ahead of time. Completing these forms online will allow us to attend to your medical needs more quickly than completing them on your arrival. Payment Options
For your convenience, we accept cash, personal checks, money orders, and most major credit cards. Payment is expected at the time services are performed. Feel free to contact our office If your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.
Dental Insurance
Our office is happy to help you file your insurance to receive the dental benefits that you and your employer are paying premiums for. Dental benefit plans can vary from company to company with different procedures covered or not covered. Insurance companies base the amounts that they will pay toward your dental treatment on restricted fee schedules related to premium payments and geographical location. In other words, your insurance plan will pay only what it allows for each service regardless of what the actual fee might be. Deductibles and co-payments are typically built in to most plans and the required payment is strictly regulated by law. As we do not have access to specific benefit information, we can only estimate benefits based on what a “typical” dental plan provides. Your estimated portion is due in full the day of treatment. Our estimates are subject to final approval by your insurance company and could change the amount due to our office. Your Employee Benefits Director can usually help you become familiar with your plan maximum and its’ restrictions, and our office will assist you in maximizing your benefits.
